I, the parent/guardian of the registering player, by applying to participate in the Washington Township Parks & Recreation Sports Program, hereby give my approval to his/her participation in any and all related activites during the current sport season. I assume all risks and hazards incidental to such participation including transportation to and from the activities. I do herby waive, release, absolve indemnity and agree to hold harmless, Washington Township, the organizers, sponsors, supervisors, or participants, and persons transporting my son or daughter to or from activities, for any claim arising out of injury to my son or daughter except covered for by accident or liability provided for by the program.
I will furnish a certified birth certificate of the above named candidate upon request of any League Official.
NOTE: All injuries to participants occurring during or to and from any scheduled athletic function must be reported to the Parks & Recreation Department immediately or the accident insurance is voided. 24 HOUR MAXIMUM
I also acknowledge that Washington Township soccer abides by the Code of Conduct, Township Ordinance. #21-2002. For the safety of our children, the Township is enforcing its Ord. #38-2007. If I become a volunteer and come in contact with children, I shall be subject to a criminal history record background check.
